A comprehensive template and guide for referring high-risk fracture patients
Patients with a recent fragility fracture are at highest risk for another fracture, particularly within the first 12-24 months. However, less than 20% of fracture patients receive osteoporosis treatment post-fracture.1
Many patients go untreated or receive suboptimal therapy after fracture, due to lack of a structured referral process. Timely identification and referral of these patients is critical to prevent further fractures and ensure appropriate therapy selection.2
The High-Risk Fracture Referral Template is designed to support primary care providers (PCPs) in efficiently identifying and referring high risk fracture patients for consideration of anabolic therapy. It was developed by an esteemed Steering Committee and based on Canadian guidelines.2,3
