About the Program

Take Charge of CKD in T2DM  is an engaging and innovative In-Room Section 3 educational program with a customized action plan that will leave participants with personalized strategies to optimize patient care and Take Charge of CKD in T2DM.

Program Learning Objectives

  • Prioritize cardiorenal protective strategies in the management of patients with type 2 diabetes (T2D) and chronic kidney disease (CKD)
  • Identify patients at high risk in whom cardiorenal protection is most important
  • Recognize knowledge gaps and construct a plan to integrate recent evidence into practice
  • Establish roles in multidisciplinary teams to optimize the management of CKD and T2DM


This activity is an Accredited Self-Assessment Program (Section 3) as defined by the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada and approved by The Canadian Society of Nephrology. You may claim a maximum of 1.5 hours (credits are automatically calculated).
