CLINICAL CASE

64-year-old female

Overweight, Non-smoker

Patient History: Anterior MI and LAD stent two years ago

Medication prior to inferior STEMI: Treated with atorvastatin 80 mg, ezetimibe 10 mg, ASA 81 mg od, Perindopril 8 mg daily. She reports compliance with her lipid lowering medication

History of Present Illness:

  • Admitted for inferior ST-segment elevation MI 4 months ago
  • Cardiac catheterization revealed near-total occlusion of the right coronary artery with diffuse atherosclerotic disease. A drug-eluting stent was placed in the right coronary artery

Current Presentation:

  • 8 weeks after her second MI, lipid profile from primary care physician (performed 2 weeks previously) : Lipid profile: LDL= 2.1 mmol/L; fasting TG = 1.3 mmol/L
  • LV dysfunction with EF 65%
