Self-Assessment Program Description
- This program is being developed for Canadian Cardiologists
- The program has been submitted to the Canadian Cardiovascular Society with a request for Section 3 Maintenance of Certification (MOC)
- In accordance with Section 3 requirements, participating physician must complete the Self-Assessment for a minimum of 10 patients
- The Self-Assessment Program will be developed and housed on a secure web-based platform (available in English and French)
- All data entry will be submitted electronically
Self-Assessment Program Objectives
- EXAMINE practice patterns and identify gaps in the management and treatment of high-risk dyslipidemia patients
- IDENTIFY post-MI patients who are not at guideline-recommended LDL-C targets and consider strategies to get these patients to target
- ASSIST clinicians in implementing strategies for their high-risk patients in order to improve the proportion of patients achieving guideline-recommended LDL-C targets
